💼 About This Role

You'll provide home health occupational therapy to adult and geriatric patients in Columbia County. You'll develop individualized care plans and help clients achieve independent living through environmental adaptations and equipment recommendations.

🎯 What You'll Do

• Perform evaluations of patients' functional status and ADL performance.
• Develop individualized intervention plans to achieve favorable outcomes.
• Evaluate home environment for hazards and identify equipment needs.
• Educate patients and families on rehabilitative care and independent living.

📋 Requirements

• Current Pennsylvania Occupational Therapist license.
• Minimum one year of recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program.
