💼 About This Role

You'll provide primary and consultative care to geriatric patients in a Federally Qualified Health Center, leading an interdisciplinary team to address complex medical and social issues. This role focuses on improving quality of life for elderly populations facing complex health challenges.

🎯 What You'll Do

• Provide direct medical care to geriatric patients including acute and chronic conditions
• Lead the SAGE interdisciplinary team (nurse and community health worker)
• Conduct routine screenings, preventive care, and health assessments
• Collaborate with outside specialists and interdisciplinary team members

📋 Requirements

• MD or DO with board certification in internal medicine or family medicine
• Valid California state medical license and DEA registration
• Experience caring for patients with geriatric issues like cognitive impairment and polypharmacy
• Strong communication skills with older adults and families

✨ Nice to Have

• Fellowship training in Geriatrics or 2+ years geriatric experience
• Bilingual in Chinese (Mandarin or Cantonese) or Vietnamese
• Proficiency with EPIC EHR
