Description

You will provide mobile primary care to seniors in various settings, including homes, assisted living, and skilled nursing facilities, ensuring patient continuity and supporting new market expansion. This role offers autonomy, strong clinical support, and leadership-track potential.

Requirements

• Active, unencumbered Nurse Practitioner (NP) or Physician Assistant (PA) license
• 2+ years experience in geriatrics, primary care, home health, SNF, or senior living preferred
• Comfortable practicing independently in mobile/community settings
• Strong clinical judgment and panel management skills
• Valid driver's license and ability to travel within region

Responsibilities

• Provide mobile primary care to seniors in homes, boarding cares, ALF, SNF, and hospice settings
• Step into established panels to ensure seamless patient continuity
• Support new market expansion and building launches
• Manage chronic conditions (HTN, CHF, COPD, diabetes, dementia, polypharmacy)
• Collaborate with interdisciplinary partners (psychiatry, palliative care, neurology)
