💼 About This Role

You'll perform home health visits for adult and geriatric patients in assisted living communities, working one-on-one to improve their functional independence. You'll be an integral member of a multi-disciplinary team providing rehabilitative care so clients can remain at home.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients' functional status.
• Develop individualized intervention plans to achieve favorable outcomes.
• Educate patients and family members in rehabilitative care.

📋 Requirements

• Current license as an Occupational Therapist in NJ.
• Minimum one year of recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program.
• Comfort working with clients who have intellectual or developmental disabilities.
