Description

You will plan, organize, and direct hospice care for patients and their families, conducting comprehensive assessments, developing care plans, and coordinating with the interdisciplinary team to provide compassionate end-of-life support. This role involves hands-on nursing care, patient education, and communication with physicians and family members.

Requirements

• Registered Nurse (RN) license
• Experience in hospice, community health, or home care
• Strong clinical assessment and care planning skills
• Effective communication and patient education abilities
• Ability to work independently and as part of a team

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family needs
• Develop and revise individualized care plans based on nursing diagnosis
• Administer medications and treatments as prescribed
