💼 About This Role

You'll provide home health visits for adult and geriatric patients, conducting evaluations and developing treatment plans for speech, language, and swallowing difficulties. As part of a multi-disciplinary team, you'll enable patients to receive care at home.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations for speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluation and physician's orders
• Educate patients and family members on rehabilitative care and activities

📋 Requirements

• Current license as a Speech Language Pathologist in North Carolina
• Certificate of clinical competence in speech from ASHA
• Solid computer skills for documentation
