You'll join a mission-driven home health team as a Physical Therapist, providing one-on-one care in patients' homes. You'll help clients regain independence through personalized treatment plans in a supportive, new-grad-friendly environment with structured mentorship.

🎯 What You'll Do

• Perform mobility, strength, and balance assessments
• Create individualized treatment plans using evidence-based interventions
• Provide gait training, manual therapy, and therapeutic exercise
• Document care using an intuitive EMR system

📋 Requirements

• Current Arizona PT license or eligibility
• Graduation from an accredited PT program
• Comfort with EMR systems (training provided)
