Description

You will provide outpatient home health visits to adult and geriatric clients in independent and assisted living communities throughout York County. You will perform diagnostic tests, develop care plans, and educate clients to promote independence and safety.

Requirements

• Current PA Physical Therapist license
• Minimum 1 year recent work experience as a Physical Therapist
• Graduation from APTA-accredited program
• Strong interpersonal, organizational, and problem solving skills
• Solid computer skills; EMR experience preferred

Responsibilities

• Make outpatient visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, balance, etc.)
• Develop and implement individualized care plans
• Educate clients and family on rehabilitative care
• Document observations and interventions using a tablet
