Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in senior living communities, performing home health visits as part of a multi-disciplinary team. Your role involves developing individualized care plans, educating clients and families, and using a state-of-the-art touch pad tablet for documentation, helping clients remain safely at home.

Requirements

• One year prior clinical experience as a licensed Physical Therapist

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, balance, strength, etc.)
• Implement integumentary protective interventions for wound prevention and repair
• Develop and implement individualized care plans including manual therapy and gait training
• Document observations and interventions using state-of-the-art touch pad tablet
