💼 About This Role

You'll join a multi-disciplinary team providing rehabilitative care to adult and geriatric patients in their homes throughout Franklin, Granville, and Vance counties. You'll conduct evaluations and develop individualized intervention plans to improve functional independence. 🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of functional status and ADL/IADL performance.
• Develop individualized intervention plans to achieve favorable outcomes.
• Assess home environment for hazards and identify equipment needs.

📋 Requirements

• Current North Carolina Occupational Therapist license.
• Minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program as indicated by transcript or diploma.

✨ Nice to Have

• Prior home care experience preferred.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
