You'll conduct comprehensive telehealth assessments for newly enrolled Medicare clients, establishing the clinical foundation for ongoing care. Your work directly shapes care strategies reducing hospital readmissions. You'll join a founding clinical team building geriatric care infrastructure.

• Conduct comprehensive telehealth assessments for newly enrolled Medicare clients
• Synthesize complex medical histories and identify clinical priorities
• Document clinical findings to inform care manager intervention strategies
• Provide clinical oversight and collaboration with care management team

📋 Requirements

• MD or DO (licensed in MD, PA, MA, CA, FL, or NJ)
• Experience in geriatric medicine, primary care, or chronic disease management
• Confidence in telehealth assessment and building clinical rapport
