💼 About This Role

You'll coordinate restorative and rehabilitative occupational therapy services as part of an interdisciplinary team for a PACE program. You'll build long-term relationships with participants, working collaboratively to maximize independence and safety. This role offers work/life balance with no after-hours documentation and lower patient volumes.

🎯 What You'll Do

• Conduct OT assessments in facility and occasionally in home
• Participate in IDT meetings to develop care plans
• Deliver and document interventions per care plans
• Provide exercise, muscle re-education, gait, and ADL training
• Train and guide Therapy Assistants on Functional Maintenance Programs

📋 Requirements

• Bachelor's degree in occupational therapy
• Valid California Occupational Therapy License or proof of eligibility
• CPR and First Aid certification within 180 days of hire
• Two years in a clinical setting with frail or elderly population preferred

✨ Nice to Have

• Master's degree in occupational therapy
• Outpatient, acute, inpatient, or skilled nursing experience
• CPR and First Aid certification upon hire
