Description

You will provide comprehensive hospice services, including patient assessments, care planning, and medication management, in collaboration with a supervising physician and interdisciplinary team. Your work ensures patients and families receive compassionate, dignified end-of-life care.

Requirements

• Registered professional nurse with advanced graduate education and clinical training
• Compliance with state Nurse Practice Act
• Experience in hospice or palliative care preferred

Responsibilities

• Provide medical care and manage terminal illness under physician supervision
• Complete history and physical examinations, order and interpret diagnostic tests
• Develop individualized plan of care and prescribe medications/treatments
• Perform hospice face-to-face visits for recertification eligibility
• Collaborate with interdisciplinary team and healthcare practitioners
