Description

As a Physical Therapist with BAYADA Home Health Care, you will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, helping them remain independent. You'll work within a multi-disciplinary team, performing assessments, developing care plans, and educating clients and families. Requirements

• One year prior clinical experience as a licensed Physical Therapist
• Valid Physical Therapist license in Vermont
• Ability to travel to client homes in Windsor County
• Strong documentation and communication skills

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements, including range of motion, transfer status, and balance
• Implement wound prevention and repair interventions using evidence-based dressings
• Develop and implement individualized care plans with manual exercises and gait training
• Educate clients and families on rehabilitative care for health and safety
