You'll provide one-on-one coaching to help patients implement lifestyle changes, working closely with primary care providers. Your core impact is reducing avoidable emergency room visits and improving health outcomes. You'll build long-term relationships with patients over multiple years.

🎯 What You'll Do

• Conduct one-on-one coaching sessions in homes, clinics, or community settings
• Collaborate with supervising Nurse Practitioner on patient case reviews
• Teach patients about health benefits and connect them with community resources
• Document patient visits and interactions per organizational guidelines

📋 Requirements

• 2+ years experience in customer service, community organizations, or teaching
• High School diploma or GED
• Residence or prior long-time residence in Springfield, IL community
• Comfortable with technology including communication and documentation systems

• Prior experience using technology systems to document customer interactions
