You'll provide skilled services to help individuals live safely and productively in their homes, as a member of the interdisciplinary care team. You'll serve as an educator and patient advocate, addressing home safety, medication management, and more. 🎯 What You'll Do

• Evaluate and address home safety and medication management
• Provide wheelchair seating and positioning interventions
• Educate patients and families on fall prevention and chronic disease management

📋 Requirements

• Current unrestricted CA Occupational Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
