You'll provide home health visits as part of a multi-disciplinary team, helping clients receive rehabilitative care at home. You'll make an impact by developing individualized care plans and promoting independence.

🎯 What You'll Do

• Perform diagnostic tests and measurements of mobility and strength
• Implement wound prevention and repair interventions
• Develop and adjust individualized care plans
• Educate clients and families on rehabilitative care

📋 Requirements

• Current MD Physical Therapist license
• Minimum one year recent work experience as a Physical Therapist
• Graduation from an APTA-accredited program
