💼 About This Role

You'll act as a liaison and educator to patients, families, and providers regarding hospice and palliative care options, coordinating seamless transitions from hospital to home. You'll also provide direct patient care to inpatient hospice patients, ensuring comfort, dignity, and quality end-of-life care.

🎯 What You'll Do

• Coordinate care transitions from hospital to hospice
• Complete admission process for hospice-eligible patients
• Provide daily General Inpatient visit services
• Educate providers on hospice qualifications and resources

📋 Requirements

• Registered Nurse (RN) with current MO licensure
• Minimum one year experience in hospice
• Acceptable pre-employment assessment results
• Ability to read, write, and communicate in English
