💼 About This Role

You'll provide continuity of care for seniors when the PACE center is closed, handling after-hours calls and resolving logistical issues. You'll build relationships with participants rather than providing short-term care. This fully remote, flexible part-time role offers a holistic care team environment.

🎯 What You'll Do

• Handle inbound/outbound calls and respond within scope of practice
• Coordinate telehealth meetings between participants and clinicians
• Troubleshoot logistical issues like transportation and medication delivery
• Support care transitions between hospitals and skilled nursing facilities

📋 Requirements

• Graduate of an Accredited School of Nursing
• Unencumbered RN license
• Nursing knowledge to treat frail elderly participants
• Ability to work assigned shifts including weekends and holidays
