You'll coordinate in-home care assessments and oversee personal care for seniors. Your work ensures participants thrive through comprehensive medical and community-based services. This role offers hybrid flexibility and close collaboration with a supportive interdisciplinary team.

🎯 What You'll Do

• Coordinate incoming calls related to participants and services.
• Communicate with participants and coordinate with care teams.
• Track and monitor home care delivery and scheduling hours.
• Assist with staffing, scheduling, and marketing enrollment support.

📋 Requirements

• Healthcare/medical licensure or equivalency.
• 3 years of professional experience.
• 3 years of case management or nursing in clinical or home setting with frail/elderly.
• Nursing knowledge for treating frail elderly participants.
