Description

You will provide home visits and on-call support for senior living communities, following care plans, administering medications, and monitoring client conditions. This role offers flexibility with a 7 on, 7 off rotation and the opportunity to work with a supportive team.

Requirements

• Current RN license in Virginia
• Minimum 1 year recent clinical experience
• Graduation from accredited nursing program
• Ability to work independently and manage time
• Strong interpersonal and computer skills

Responsibilities

• Follow designated care plan per patient's needs
• Make home visits in designated geographic territories
• Administer medication, wound care, treatments, and procedures
• Monitor client conditions and report changes
• Perform client assessments and case management
