Description

You will make home visits to adult and geriatric patients in Plymouth and surrounding towns, following a designated care plan to administer medication, provide wound care, and perform treatments while monitoring clients' conditions and coordinating with the clinical team.

Requirements

• Current Registered Nurse license in Massachusetts.
• Graduation from an accredited nursing program.

Responsibilities

• Follow designated care plans and administer medications, wound care, treatments, and procedures during home visits.
• Monitor client conditions and report changes to the Clinical Manager.
• Perform client assessments and RN case management coordination.
• Document observations, interventions, and evaluations accurately using a touch pad tablet.
• Follow up on and execute doctors' orders.
