You'll deliver patient-centered health coaching and chronic disease management via chat, phone, and video. You'll triage symptoms, provide education on diabetes and hypertension, and coordinate care with cross-functional teams. 🎯 What You'll Do

• Deliver patient-centric health coaching integrating medical needs with preferences.
• Triage patient-reported symptoms and side effects per internal protocols.
• Provide diabetes, hypertension, and weight management education via chat, phone, video.
• Coordinate care between patients, providers, and support teams.

📋 Requirements

• Active Registered Nurse license in good standing.
• Bachelor of Science in Nursing (BSN) required.
• Certified Diabetes Care and Education Specialist (CDCES) or willingness to pursue.
• 3+ years of direct patient care in an ambulatory or telehealth setting.
