Description

Provide compassionate, high-quality home health care to adult and geriatric clients in the Boston area. You will follow care plans, administer medication, perform wound care, monitor conditions, and coordinate care, all while helping clients remain at home with dignity.

Requirements

• Current Registered Nurse license in Massachusetts
• Graduation from an accredited nursing program
• Prior clinical experience as a licensed RN in sub-acute or acute environment

Responsibilities

• Follow designated care plans and perform home visits in assigned geographic territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to clinical manager
• Execute and document doctors' orders accurately
• Perform client assessments and case management coordination
