Description

You will provide personal care and support to hospice patients and their families, working as part of a multidisciplinary team. Your role includes observing and reporting changes in patient condition, assisting with feeding, and participating in care coordination meetings.

Requirements

• Minimum age 18
• CNA/STNA certification on Ohio registry, active
• CPR required
• 1+ year direct patient care experience in institutional or home care
• Valid driver's license and insured reliable transportation

Responsibilities

• Provide personal care with respect and dignity
• Maintain a positive, professional manner
• Record STNA plan of care for each patient
• Observe, record, and report patient's care and symptoms
• Communicate appropriately with patient/family and agency personnel
