💼 About This Role

You'll provide skilled nursing and rehabilitative care to clients in their homes in Erie County. As a home care nurse, you'll be an integral member of a multi-disciplinary team ensuring clients can remain at home. Case management and accurate documentation using electronic medical records are key to the role.

🎯 What You'll Do

• Making home visits to clients in designated territories
• Administering medication, wound care, treatments, and procedures
• Monitoring clients' conditions and reporting changes
• Performing client assessments and coordinating care

📋 Requirements

• Current LPN license in Pennsylvania
• Minimum one year of recent clinical experience as an LPN
• Graduation from an accredited nursing program
• Ability to travel to cases as assigned
