💼 About This Role

You'll provide home health visits, assessments, and admissions for adult patients in the Seaford, Laurel, and Georgetown area. As a home care RN, you'll be an integral member of a multi-disciplinary team delivering skilled nursing and rehabilitative care. This role offers the opportunity to work independently and make a direct impact on patient well-being.

🎯 What You'll Do

• Follow a designated care plan in accordance with patient needs
• Perform home visits to clients in designated geographic territories
• Administer medication, wound care, treatments, and procedures
• Document observations, interventions, and evaluations using a touch pad tablet

📋 Requirements

• Current Registered Nurse license in Delaware
• Minimum 1 year of recent clinical medical/surgical experience
• Graduation from an accredited nursing program
• Ability to travel to cases as assigned
