Description

You will perform home health visits for adult and geriatric patients in Franklin County, VT, as part of a multi-disciplinary team that provides rehabilitative care to help clients remain at home.

Requirements

• Current Vermont Occupational Therapist license
• At least one year of recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations of patients' functional status and ADL/IADL performance
• Develop individualized intervention plans and assess home environment for hazards
• Identify equipment needs and environmental adaptations
• Accurately document observations and interventions using a touch pad tablet
