Description

You will make home visits to provide skilled nursing and rehabilitative care to adult clients in a home setting, including medication administration, wound care, and monitoring conditions. You'll be part of a multi-disciplinary team helping clients remain at home, using electronic medical records on a touch pad tablet.

Requirements

• Current LPN license in Colorado
• Minimum 1 year recent clinical experience as an LPN
• Graduation from an accredited nursing program
• Ability to travel to cases
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to managers
• Perform client assessments and case management
• Document observations and care using electronic medical records
