💼 About This Role

You'll lead a team of RN Case Managers and LVNs, coordinating patient care and ensuring clinical excellence in a home health setting. You'll supervise field clinicians, conduct training programs, and meet with customers to improve patient outcomes. This hybrid role offers 3 days in the field and 2 days in the office.

🎯 What You'll Do

• Supervise field clinicians providing home health care
• Coordinate training and educational programs for nurses
• Conduct bi-weekly case conferences with clinical team
• Review patient diagnoses and assist in care plan development

📋 Requirements

• Two years' management or supervisory experience in health care
• Graduate of an accredited school of nursing
• Current CA Registered Nurse License (RN)
• Current Basic Life Support (BLS) card
