You will work with clients in their homes, crafting treatment plans and applying innovative techniques to help them achieve their goals. As an integral member of a multi-disciplinary team, you provide skilled nursing and rehabilitative care that enables clients to remain at home.

Requirements

• Current Physical Therapist license in Hawaii
• Graduation from an APTA or CAAHEP accredited program
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Perform diagnostic tests and measurements for mobility, range of motion, strength, balance, and safety
• Develop and implement individualized care plans including manual therapy, gait training, and balance interventions
• Implement integumentary protective interventions for wound prevention and repair
• Continually assess and revise the physical therapy care plan
• Educate clients and families on rehabilitative care and activities to promote health and independence
