💼 About This Role

You'll provide skilled, compassionate care in participants' homes, collaborating with an interdisciplinary team to coordinate care plans. This role focuses on patient care without OASIS assessments, earning hourly pay that includes travel time. You'll enjoy flexible scheduling and built-in documentation time.

🎯 What You'll Do

• Conduct home health nursing assessments to determine equipment needs.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document nursing interventions as agreed upon in care plans.
• Instruct participants and families on disease process and self-care techniques.

📋 Requirements

• Graduate of an accredited school of nursing
• Unencumbered RN License required
• Minimum one year nursing experience with frail or elderly population
• Reliable means of transportation

• Home health agency experience preferred
• CPR and First Aid certification upon hire preferred
