Requirements

• Licensed Practical Nurse (LPN) license
• Ability to provide advanced care (trach/vent) or willingness to train
• Compassionate and reliable
• Valid driver's license and reliable transportation

Responsibilities

• Provide one-on-one nursing care to clients in their homes
• Manage tracheostomy and ventilator care as needed
• Document patient care using electronic charting
• Adhere to care plans and communicate with clinical support
