🎯 What You'll Do

• Manage a diverse caseload and coordinate patient care.
• Develop and implement individualized care plans.
• Coach and support Certified Nursing Assistants (CNAs).
• Maintain accurate electronic charting within 24 hours.

📋 Requirements

• Associate's or Baccalaureate degree in Nursing.
• Current valid RN license in Indiana or compact state.
• 2 years of clinical RN experience.
• Reliable transportation, valid driver's license, and auto insurance.

✨ Nice to Have

• 1 year of home health experience.
• Case management or supervisor experience.
• Knowledge of Medicaid paid family caregiving program.
