💼 About This Role

You'll coordinate care for children with chronic conditions in a virtual, multidisciplinary team. You'll ensure seamless scheduling, education, and medication management for families. This role offers flexible part-time hours and fully remote work.

🎯 What You'll Do

• Coordinate and schedule clinical appointments across the care team
• Provide evidence-based education to families on chronic condition management
• Initiate and manage medication prior authorization requests
• Document clinical interactions accurately in EHR platforms

📋 Requirements

• Active Registered Nurse (RN) license in good standing
• Proficiency with EHR systems and multiple technology platforms
• Ability to work collaboratively in a remote environment
• Experience with diverse pediatric populations

• Bilingual English/Spanish
• Experience in pediatric chronic care or care coordination
• Familiarity with prior authorization processes
