💼 About This Role

You'll provide nursing care to seniors in a value-based PACE program. You'll be the primary voice on an interdisciplinary team, conducting assessments and managing complex care. This role offers the chance to travel and work across multiple markets.

🎯 What You'll Do

• Conduct required nursing assessments every 6 months
• Develop and implement discipline-specific plan of care
• Coordinate care for high-complexity participants across settings
• Act as triage point for clinic management and ER diversion

📋 Requirements

• Graduate of an accredited school of nursing
• Unencumbered RN license
• Minimum 2 years nursing experience with frail or elderly population
• Ability to travel up to 100% of the time

• CPR and First Aid certification upon hire
