Description

As part of the High Risk team, you will visit patients in their homes to facilitate telehealth visits with a provider, perform clinical assessments, and coordinate care. Your work helps improve access to primary care and supports patients in managing their health after hospital stays.

Requirements

• 1+ year home-based healthcare or full-time EMT experience
• Certified Medical Assistant (MA) and BLS preferred (or obtain within 6 months)
• Strong written and verbal communication skills
• Proven ability to build rapport and trust with patients
• Proficiency in computer technology and video platforms

Responsibilities

• Facilitate telehealth visits in patient homes using video technology
• Conduct patient assessments and document findings for provider
• Perform clinical duties: vitals, EKGs, specimen collection, spirometry, retinopathy
• Coordinate care with patients, families, and outside care teams
• Teach patients and families to use technology for care coordination
