You will manage client communication and coordination with referral sources, including providing treatment updates and planning discharges. Your role ensures a seamless experience for clients and their support systems, helping them transition from virtual behavioral health treatment to the next steps in their care.

Requirements

• Bachelor's degree in health sciences, communications, psychology, social work or related field
• Minimum 2 years of relevant work experience (healthcare, case management, discharge planning, or referral relations)
• Strong interpersonal, relationship-building and listening skills
• Metrics- and results-oriented mindset with experience working against targets
• Proficient in Salesforce and Google Suite/MS Office

Responsibilities

• Provide treatment updates to referring providers
• Develop and implement discharge plans
• Make accurate and timely referrals to aftercare resources
• Document case management contacts in progress notes
• Collaborate with internal stakeholders and outreach teams
