Description

You will perform home health visits for adult and geriatric clients in designated territories, providing rehabilitative care and skilled nursing to help them remain at home. As a Physical Therapist, you'll conduct diagnostic tests, develop individualized care plans, and educate clients and families on health and safety. You'll be part of a multidisciplinary team, documenting care using a touch pad tablet.

Requirements

• A current Maryland Physical Therapist license
• A minimum of one year of recent work experience as a Physical Therapist
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem solving skills
• Solid computer skills; prior experience with electronic medical records (EMR) preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements
• Implement integumentary protective interventions to prevent wound formation and promote wound repair
• Develop and implement appropriate individualized care plans
• Continually assess and revise the Physical Therapy care plan
• Educate and instruct clients, family members, or other client representatives
• Accurately document observations, interventions, and evaluations
