💼 About This Role

You'll make home visits to provide speech therapy evaluations and interventions for adult and geriatric clients in Nassau County, FL. You'll develop treatment plans and collaborate with a multidisciplinary care team to promote client health and independence. This role offers full-time schedules and the support of an accredited home health care provider.

🎯 What You'll Do

• Perform evaluations for speech, language, communication, or swallowing difficulties.
• Develop and revise treatment plans based on evaluations and physician orders.
• Educate patients and families on rehabilitative care and activities.
• Document observations, interventions, and evaluations using a touch pad tablet.

📋 Requirements

• Current Speech Language Pathologist license in FL.
• Certificate of Clinical Competence in Speech from ASHA, or equivalent education and experience.
• Solid computer skills and prior experience with electronic medical records.
