Description

As a Clinical Manager, you will conduct client assessments, develop care plans, coordinate communication among clients and staff, supervise Home Health Aides, and facilitate training. You'll make a direct impact on geriatric clients staying safely at home.

Requirements

• Registered Nurse (RN) with 2+ years experience
• Home care experience preferred
• Supervisory experience as a Nurse Manager preferred

Responsibilities

• Visit geriatric clients and conduct assessments
• Develop and execute client care plans
• Coordinate communications between clients, field staff, office and clinicians
• Supervise and support Home Health Aide staff in the field
• Facilitate new hire orientation and training for field staff
