This entry-level role focuses on follow-up visits under the supervision of a Physical Therapist Case Manager.

🎯 What You'll Do

• Provide hands-on therapeutic interventions like gait training and manual therapy.
• Conduct detailed patient evaluations in the home environment.
• Implement individualized plans of care under supervision.
• Document patient care within 24-48 hours.

📋 Requirements

• Current California PT License.
• Current BLS/CPR Certification.
• Valid CA Driver's License with clean record.
• Graduate of accredited Physical Therapy program.

✨ Nice to Have

• One year of experience as a Physical Therapist.
• Prior home health experience.
• Experience treating high acuity patients.
