Description

You will conduct in-home assessments and provide personalized recommendations for home modifications to help older adults and individuals with disabilities live safely and independently. This role involves traveling to client homes, collaborating with a remote team, and using digital tools for documentation.

Requirements

• Degree in Occupational Therapy from an accredited program
• 2+ years of clinical experience
• Strong interpersonal and clinical reasoning skills
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation

Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver personalized, evidence-based recommendations for home modifications
• Educate clients and caregivers on fall prevention and safe mobility techniques
• Engage clients and families in collaborative care planning
