Description

You will provide home-based physical therapy to patients in Philadelphia, developing care plans and helping them regain independence after hospital discharge.

Requirements

• Active Physical Therapist license in Pennsylvania
• Ability to travel to patient cases
• Strong assessment, communication, and active listening skills
• Compassionate, reliable, and patient-centered approach

Responsibilities

• Perform diagnostic tests and measurements for mobility, range of motion, strength, balance, and safety
• Develop and implement individualized care plans including therapeutic exercises, gait training, and balance interventions
• Continually assess and revise care plans, collaborating with nursing in multidisciplinary approach
• Implement integumentary protective interventions to prevent wounds and promote wound repair using evidence-based dressings
