Description

You will conduct in-home assessments and deliver personalized, evidence-based recommendations for home modifications to help older adults and individuals with disabilities live safely and independently. You'll educate clients on fall prevention, collaborate with a Director of Clinical Operations, and maintain documentation through an electronic system.

Requirements

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Experience in home health, aging in place, or disability services preferred
• Reliable transportation and valid driver's license

Responsibilities

• Conduct in-home assessments and functional evaluations
