💼 About This Role

You'll provide rehabilitative care to adult and geriatric patients during home health visits in Middlesex County. Your assessments and intervention plans will enable clients to live independently at home. You'll be part of a multi-disciplinary team with PPE support and training opportunities.

🎯 What You'll Do

• Make home visits to clients in designated territories
• Perform evaluations of functional status and develop intervention plans
• Assess home environment for hazards and recommend adaptations
• Educate patients and families on rehabilitative care

📋 Requirements

• Current New Jersey Occupational Therapist license
• Minimum one year of recent OT work experience
• Graduation from an accredited occupational therapy program
