💼 About This Role

You'll serve as a core clinical member of the interdisciplinary team, conducting assessments and managing care plans for frail elderly participants in a PACE program. You'll deliver hands-on nursing care and triage urgent needs in the clinic. This role offers the opportunity to build meaningful relationships with a culturally-focused senior care model.

🎯 What You'll Do

• Conduct comprehensive nursing assessments for new enrollees and ongoing participants.
• Develop and maintain individualized care plans in collaboration with the interdisciplinary team.
• Triage inbound communications and prioritize clinical needs.
• Administer medications, vaccinations, and wound care within RN scope.

📋 Requirements

• Valid and active California RN license, unencumbered.
• 2+ years of RN experience.
• Experience conducting nursing assessments and contributing to care plan development.
• Comfort managing a complex, multi-condition patient population with clinical autonomy.

✨ Nice to Have

• Background in geriatrics, long-term care, urgent care, or home health.
• Proficiency in Mandarin and/or Cantonese.
• Experience with electronic health records.
