Description

As a per diem Medical Social Worker, you'll provide in-home care to patients in Fitchburg, Leominster, Lunenburg, Westford and surrounding towns. You'll conduct clinical social evaluations, coordinate community resources, and advocate for patients as part of a multi-disciplinary team, helping them receive medical care at home.

Requirements

• Master's Degree in Social Work
• Licensed Clinical Social Worker (LCSW or LICSW) in Massachusetts
• Prior home care experience preferred

Responsibilities

• Conduct home visits and provide clinical social evaluations
• Plan interventions based on evaluation findings
• Identify and coordinate community resource referrals
• Advocate for clients and families navigating community systems
• Participate in developing total plan of care with multidisciplinary team
