Description

You will provide home health physical therapy services to patients in Bucks County, PA, developing and implementing individualized care plans, performing diagnostic tests, and educating patients and families to promote independence and health.

Requirements

• Active PT license in Pennsylvania
• Ability to travel to cases as assigned
• Strong assessment and communication skills
• Compassionate patient care approach

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements (mobility, range of motion, balance, etc.)
• Implement wound prevention and repair interventions using evidence-based dressings
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate and instruct clients and families on rehabilitative care and activities
