Description

You will conduct in-home assessments and provide personalized home modification recommendations to help older adults and individuals with disabilities live safely and independently. Collaborate with a remote clinical team and use technology for documentation.

Requirements

• Degree in Occupational Therapy from accredited program
• Active, unencumbered OT license
• 2+ years clinical experience
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation

Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver personalized home modification recommendations
• Educate clients and caregivers on fall prevention and safe mobility
• Engage clients and families in collaborative care planning
• Maintain accurate documentation through electronic system
